What to Bring to Your Intake Appointment

Thank you for your interest in The Arc of Schuyler! We look forward to meting you and getting you started in the process to receiving services.

The following documents will be helpful to have at your intake meeting:
  • Individualized Service Plan (most recent - within 12 months)
  • Complete Physical Exam (within the past 12 months; including list of medications)
  • PPD/TB test (within the past 12 months)
  • Chicken Pox/Varicella immunization or written results of the Varicella tier
  • DDP-2
  • DDP-4 (For Day Service needs)
  • Psychological evaluation including an IQ score and an adaptive behavior assessment
  • Psychosocial evaluation
  • Copies of Social Security, Medicaid, and Private Insurance cards
  • Age of onset documentation if the person is over age 22
Please bring the following documents if applicable:
  • Most recent Individualized Education Plan
  • Psychiatric Evaluation
  • Current Behavior Plan
  • DDSP Risk Assessment
  • Program and Plans and IPOP
  • DVE/CWBA (if applying for a vocational program)
  • Clinical evaluations
  • Legal Guardian Paperwork
  • Current SSI/SSD Award letter
  • Community Habilitation Authorization Forms

The Developmental Disabilities Service Office (DDSO) may request additional documentation regarding the person’s disability. A service coordinator can help you with gathering and submitting this information for approval.
